Complete Arcade mode with Itachi.

Complete Story mode under the Hard difficulty setting.

Complete Story mode without using any continues.

Complete Arcade mode with the default characters (Naruto, Sasuke, Sakura, Kakashi, Rock Lee, Shikamaru and Neji).

Successfully complete Arcade mode as Tsunade.

Successfully complete Arcade mode as Jiraiya.

Successfully complete Arcade mode with all other characters.

Complete one of the following tasks to unlock Tsunade.

Unlock Gai, Temari, and Gaara. This can be done by completing Story mode under the Normal or Hard difficulty setting without using any continues.
Successfully complete Story mode under the Difficult setting using no continues.

Press Y to change the color of the outfits.
